_{Appointments: Office visits are by appointment only and maybe scheduled by calling your Owensboro Medical Practice office. The exception is The Immediate Care Center which takes walk-ins only. New patient visits last approximately 30 mins. You will receive a telephone confirmation one work day prior to your scheduled appointment.}

_{After Hours: If you experience a minor injury or illness after our office hours.
We encourage you to visit our Immediate Care Center at 1200 Breckenridge Street in Owensboro. There is a physician on call 24 hours, please call the appropriate clinic number and ask to have a physician paged. The on-call physician will return your call as soon as possible. If you have a life-threatening situation, please call 911 or proceed to the nearest emergency room.}

_{Health Plan Participation: Our
offices participate with many networks and managed care organizations. To
determine whether our office participates with your health plan, you may ask a
member of our staff, contact your insurance company, or speak with your employee
benefits counselor.}

_{Financial Policy: We are glad
to file our charges with your insurance company as a courtesy to you. Co-pays
are due at the time of service. You are also responsible for paying any
deductibles and/or co-insurance. In the event that your insurance company denies
the claim, you will be responsible for the balance in full within 30 days of
receiving your first billing statement.}

_{If you have no insurance, payment in full is due for all services rendered at
that time. If you are unable to pay your balance in full, please contact our
billing department to negotiate the terms of your payment. The direct line to
our billing department is 270-684-0028.

Failure to comply with these terms could result in collection procedures
including the use of collections agencies and legal counsel. It could jeopardize
your future appointments with any of part of Owensboro Medical Practice.

Refund checks for credit balances of $5.00 or less will not be issued. We
accept cash, check, MasterCard, Visa and Discover payments.}
